Yes, it's one of the most demanding DOS Glide games. DOSBox takes a lot of CPU power to emulate it. Apart from lowering draw distance in game options there's not much you can do. Unless you plan to buy a faster CPU.

Try using reg file from nGlide compatibility list. The one you have is incomplete, it lacks entries for 64-bit OS.

If I remember correctly F4 switches back to fullscreen. Make sure you have "Voodoo graphics" selected in Graphics options.
