Two things: Back then, most people weren't properly aware of the patches, didn't know which release they had, nor which version. Patches weren't properly introduced, SCi even rereleased the v1.1 patch as the "XP-Vista patch" in 2003, years after v2. And that confusion even amongst the comm...

There's also an unofficial patch now based on GOG's release: http://rr2000.toshiba-3.com/R4/PC/GOGPatchV3.zip The included winmm.dll might not work in WinXP though. Ah ok, localization, so it doesn't imply any game breaking bug. Interesting to know that the US version already had the quitting dialogue.

If you have an american version Carmageddon 2 don't replace these files: DATA.TWT DATA\INTRFACE\creditslist.txt DATA\INTRFACE\Interface.txt Could you explain yourself? I don't have an american version to test myself but for example I don't see how replacing creditslist.txt could do any difference. ...

Most people in the Carmageddon community let music players running in the background, I even tend to run C2 with no sound at all... no wonder why we didn't really pay attention to the lack of CD playback! Ah I read too fast :-/... And actually I have that mouse getting lost issue as well. Really minor issue indeed. For your first issue, install the latest patch (super patch v2) and it should fix that mouse in a box problem. You can get it there http://rr2000.toshiba-3.com under PC TOOLS. I second this suggestion.
It is also interesting when creating custom content for the game, or simply running the game in its native resolution without stretching the screen.
dgVoodoo is the only glide wrapper supporting this so far.
